Waiver | Weekend Hockey Tournaments

TERMS AND CONDITIONS

The player assumes all risk of personal injury which may result from participation in all activities of Weekend Hockey Tournaments Inc(WHT). The player will not hold WHT, any of WHT staff, owners or the proprietor, or the employees of any ice facility used by WHT liable for injury which the player may sustain while participating in any WHT sports activity. The player understands that the sport of ice hockey has physical dangers which may result in serious injuries or death.

ALL PLAYERS ARE ADVISED TO CARRY MEDICAL INSURANCE

The player certifies that he/she has no known Medical condition which would prohibit him/her From playing the sport of ice hockey. The player agrees to wear specified CSA approved Equipment. The player agrees that he/she will conduct themselves in a responsible manner during all WHT activities. The player agrees to reimburse WHT in full within 5 days notice, for the cost of any property damage for which the player or team is Held responsible by the WHT staff or owners. The player agrees to WHT policy that there is no reimbursement to any player. The WHT President has the final authority as to what divisions the teams and players will participate. The player understands and agrees that he/she may be expelled from WHT for any of the following reasons.
1)Financial delinquency.
2)Falsification of registration information.
3)Failure to abide by WHT rules.
4)By signing this waiver I agree to all these terms.
In consideration of being allowed to participate in any way in the Weekend Hockey Tournaments athletic/sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Weekend Hockey Tournaments Inc their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.

I acknowledge having obtained or having adequate opportunity to obtain independent legal advise with respect to the content and legal ramifications of this Waiver.
